A title proves ownership of a vehicle. As long as you have the title to your vehicle, you can legally sell it, trade it in at a dealership, or register it in another state when you move. However, titles are easy to lose, and they can also be stolen or even damaged. All it takes is a fire, and the proof that you own your car is up in smoke. Thankfully, you can replace your lost, stolen or damaged car title in Arizona.
The state of Arizona offers two ways to replace your lost, stolen or damaged car title. You can do it in person at an MVD office, or you can do it through the mail. Both will require the same fees ($4 for a duplicate title). However, each method has some specific steps that must be followed.
To replace a lost or stolen car title in person:
- You’ll need to visit your local Arizona MVD office.
- Complete Form 96-0236 (Title and Registration Application).
- Mark “Duplicate” and “Title Only” on the form.
- Enter the vehicle mileage.
- Sign the form (only one owner can sign, even though there may be multiple owners for the vehicle)
- Bring legal photo identification
- Pay the $4 replacement/duplicate title fee
To replace a lost or stolen car title by mail:
You’ll need to complete Form 96-0236 (Title and Registration Application)
Mark “Duplicate” and “Title Only” on the form.
Sign the form (again, only one person can sign, even if there are multiple owners.
Enter the mileage on the car.
Include a check for $4 payable to the Motor Vehicle Division.
Mail the form and payment to the following address:

Note: If your title has been damaged or made illegible, the state of Arizona requires you to send the existing title with the form (or be turned in if you’re doing it in person). You do not need to sign the back of the title if you’re obtaining a duplicate/replacement title.
